A UnitedHealthcare® Nursing Home Plan is a specialized Medicare Advantage plan for long-term nursing home residents. Plan members get more benefits than Original Medicare, including Part D prescription drug coverage, as well as support from a dedicated Optum® care team.
The care team includes either a nurse practitioner or physician assistant, often supported by a registered nurse. The care team will serve as your primary point of contact for the health of your loved one and is your extra set of eyes and ears in the nursing home. They will keep you informed if there is a change in your loved one’s health status.
The care team works with everyone involved in your loved one’s care to help eliminate communication gaps and coordinate care, delivering the right care, in the right place, at the right time.
